1.     Filled in application form for academic recognition of foreign qualification;

2.      Legalised original Шаходатнома дар бораи тахсилоти миёнаи умумй  (or other equivalent document) or its certified true copy;

3.      Legalised original appendix (табели чамъбасти бахохои азхудкунй) to the Шаходатнома дар бораи тахсилоти миёнаи умумй  or its certified true copy;

4.      Passport (original or certified true copy);

5.      Proof of name change (original or certified true copy; if applicable);

6.     Other documents; the Centre has the right to ask for additional documents or information; the Centre also welcomes submission of any documents and/or information related to your studies that you may find to be relevant.

Please note that the Centre accepts documents in three languages: Lithuanian, English or Russian. If a document is issued in other language than the above, you need to provide an official translation. A translation is considered to be official when it is bound to a certified (notarized) true copy of the document in the original language and confirmed with the translator’s signature.
